Terms & Conditions of Service

Norman Family Medicine, P.C. – Terms and Conditions
By registering as a patient of Norman Family Medicine P.C., enrolling in a DPC membership, scheduling a telehealth or in-person visit, or otherwise using the services of the Practice, you ("Patient") agree to be bound by these Terms and Conditions ("Terms"). If you do not agree, you should not use our services. These Terms, together with any signed patient agreements, DPC membership agreements, and our Privacy Policy, constitute the entire agreement between you and Norman Family Medicine P.C.
These Terms apply to all patients regardless of payment model, including DPC members, cash-pay patients, and insurance-covered patients (once credentialing is complete). Special provisions applicable to specific payment models are identified below.
1. Nature of Services & Practice Model
Norman Family Medicine P.C. is a hybrid family medicine practice offering primary care services through multiple modalities:
Telehealth: Virtual visits via secure, HIPAA-compliant video platform for eligible patients and conditions.
In-Person (Upcoming): Office-based visits at our New Jersey location upon opening.
DPC Membership: A flat monthly membership model providing enhanced access to primary care services for a fixed fee, not billed to insurance.
Cash-Pay: Fee-for-service visits paid directly at the time of service or via invoice.
Insurance (Pending): The Practice is currently in the process of credentialing with health plans. Insurance billing will be available once credentialing is complete.
2. No Emergency Services
Emergency Services Disclaimer
Norman Family Medicine P.C. does NOT provide emergency medical services. If you are experiencing a medical emergency, call 9-1-1 or go to your nearest emergency room immediately. Do not use telehealth, secure messaging, the patient portal, or any Practice contact method to seek emergency care. The Practice is not responsible for outcomes that result from a delay in seeking emergency care.
After-hours care guidance will be communicated to patients as part of their onboarding. Patients in a medical emergency should always call 911 regardless of their membership or payment status.
3. Telehealth Services
By participating in Telehealth services, you acknowledge and agree to the following:
Eligibility & Appropriateness
Not all medical conditions are appropriate for telehealth evaluation. The provider retains sole clinical discretion to determine whether a condition is suitable for a virtual visit. If Telehealth is deemed inappropriate, the provider will direct you to an appropriate level of care, which may include an in-person visit, urgent care, or emergency services.
Licensure & Geographic Scope
Telehealth services are provided by providers licensed in the States of New Jersey (NJ), Pennsylvania (PA), Delaware (DE) and Georgia (GA). You must be physically located in a state where the treating provider is licensed and where the Practice is authorized to deliver telehealth at the time of your visit. It is your responsibility to confirm your physical location at the start of each telehealth encounter. Providing a false location may result in termination of service.
Technology Requirements
You are responsible for having adequate internet connectivity, a functioning device with camera and microphone, and access to our designated telehealth platform. The Practice is not liable for service interruptions caused by technical failures on your end.
Informed Consent for Telehealth
You will be asked to provide informed consent for telehealth prior to your first virtual visit. This consent acknowledges that telehealth has limitations compared to in-person care, including the inability to perform physical examinations, and that there are potential privacy and technical risks. Consent is required before each service modality begins.
Prescribing Via Telehealth
Prescribing is at the sole discretion of the treating provider. Not all medications may be prescribed via Telehealth under federal and state law, particularly controlled substances. Controlled substances — including opioids and benzodiazepines — are not prescribed via telehealth. This policy applies to all Telehealth visits regardless of diagnosis or clinical circumstance.
Prescription refills required an active patient relationship.
4. Direct Primary Care (DPC) Membership Terms
What DPC Is & Is Not
The DPC membership is a direct contractual relationship between you and Norman Family Medicine P.C. for primary care services. DPC is NOT health insurance, is NOT a substitute for health insurance coverage, and does NOT cover specialty care, hospitalization, emergency care, surgery, imaging, laboratory services outside any negotiated arrangements, or any services outside the scope of primary care as defined by the Practice.
Membership Fees & Billing
Membership fees are billed monthly (or annually, if elected) via credit card or ACH on file.
Fees are non-refundable for the current billing cycle once charged, except in cases of provider-initiated termination or documented billing error.
The Practice reserves the right to adjust membership fees with at least 30 days' written notice to members.
Failure to maintain a valid payment method may result in suspension or termination of membership.
Services Included in DPC Membership
The services included in your specific DPC tier will be outlined in your DPC Membership Agreement, which is a separate document incorporated by reference into these Terms. The Practice retains the right to modify included services with 30 days' notice.
Cancellation & Termination
Patient-initiated cancellation: You may cancel your DPC membership with 30 days' written notice. You will continue to have access to membership services through the end of the final paid billing period.
Practice-initiated termination: We reserve the right to terminate a membership for non-payment, violation of these Terms, disruptive or abusive behavior, or any reason permitted by law, with 30 days' written notice except in cases involving patient safety or egregious conduct.
Upon termination, you will be provided a copy of your medical records upon written request and a referral to another provider upon request.
DPC & Insurance Interactions
Services rendered under your DPC membership will NOT be billed to your insurance company. If you wish to have any service submitted to your insurer (once the Practice is credentialed), that service may need to be handled under a separate billing arrangement, and additional fees may apply. You assume full responsibility for understanding your insurer's policies regarding DPC arrangements.
5. Cash-Pay Services
Cash-pay patients visit is due before services. The Practice's current fee schedule is available upon request and is subject to change. We will provide you with a superbill upon request for potential reimbursement from your insurer; however, we make no guarantee that your insurer will reimburse you, and any dispute with your insurer is your responsibility to resolve. The Practice is not a party to your insurance contract.
6. Insurance Billing (Pending)
Norman Family Medicine P.C. is currently in the process of obtaining credentialing with health plans. Until credentialing is complete, the Practice cannot submit claims directly to insurers. When insurance billing is available, the following will apply:
You are responsible for providing accurate and complete insurance information. Providing false insurance information is fraudulent.
You authorize the Practice to submit claims to your insurer on your behalf and to receive payment directly from your insurer.
You are responsible for all cost-sharing amounts including copays, coinsurance, and deductibles, which are due at the time of service unless otherwise arranged.
If your insurer denies a claim, you may be responsible for the full balance. The Practice will assist in appeals where clinically appropriate but is under no obligation to pursue appeals on your behalf.
The Practice is not responsible for verifying your specific benefits. It is your responsibility to understand your coverage.
7. Medicare & Medicaid Patients
Patients enrolled in Medicare or Medicaid cannot be charged cash for covered services
These patients are declined and referred appropriately until further notice
8. Cancellation & No-Show Policy
24-hour notice required
Late cancellations or no-shows may incur a fee
9. Scope of Care & Patient Responsibilities
Scope of Care
Norman Family Medicine P.C. provides primary care services. Conditions or circumstances outside the scope of primary care will result in a referral to appropriate specialists or facilities. Referral to a specialist does not guarantee that the specialist will accept your insurance, agree to see you, or be in-network. Any care provided by referred providers is under a separate patient-provider relationship and these Terms do not apply to that care.
Patient Responsibilities
Provide accurate, complete, and updated personal, health, and insurance information.
Disclose all current medications, supplements, allergies, and relevant medical history.
Keep scheduled appointments or cancel/reschedule with at least 24 hours' notice. Repeated no-shows or late cancellations may result in a fee and/or discharge from the Practice.
Treat all staff and providers with respect. Abusive, threatening, or harassing behavior will not be tolerated and may result in immediate termination of care (with emergency services referral).
Follow through on agreed care plans, lab orders, referrals, and medications to the best of your ability, and communicate with us if you are unable to do so.
Maintain updated payment information and pay all applicable fees timely.
10. Liability Disclaimer and Service limitations
Telehealth services have inherent limitations, including the inability to perform comprehensive physical examinations and certain diagnostic procedures. If a condition requires in-person evaluation, the patient will be directed to seek appropriate local medical care.
To the fullest extent permitted by applicable law, Norman Family Medicine, P.C. disclaims liability for any unauthorized access, data breaches, or disclosure of information that may occur despite the implementation of reasonable administrative, technical, and physical safeguards.
Norman Family Medicine, P.C. reserves the right to modify or update these Terms, Conditions, and related notices at any time without prior notice.
Information and content provided through this website or services shall not be relied upon as a substitute for professional medical, legal, or financial advice. Patients are responsible for consulting appropriate licensed professionals.
SMS/text message communications are not guaranteed to be secure. Norman Family Medicine, P.C. is not responsible for any charges, errors, delays, or failures in delivery of SMS messages caused by mobile carriers or third-party service providers.
11. Intellectual Property, Governing Law & Amendments
Intellectual Property
All content on our website, patient portal, and communications — including text, graphics, logos, and materials — is the property of Norman Family Medicine P.C. and may not be reproduced, distributed, or used without our express written permission.
Governing Law & Dispute Resolution
These Terms shall be governed by and construed in accordance with the laws of the State of New Jersey, without regard to conflict of law principles. Any dispute arising from or related to these Terms or your care at the Practice shall first be subject to a good-faith attempt at informal resolution. If informal resolution fails, disputes shall be resolved in the courts of the State of New Jersey. You consent to personal jurisdiction in New Jersey.
Amendments
We reserve the right to modify these Terms at any time. Material changes will be communicated to active patients by email or written notice at least 30 days before taking effect. Continued use of our services after such notice constitutes acceptance of the updated Terms.
